Grant Reporting

Grantees are asked to submit a brief follow-up report on their grant using our online portal. To do so, simply login to our system with the same user name and password that was used to apply. If you need assistance logging in, please send us an email at [email protected]. Then please complete the "Follow Up" form attached to your grant in your organization’s records. To complete it, please click the "Start" button at the bottom right-hand corner of the grant record. The Foundation no longer accepts final grant reports in the mail.
